Student Registration Verification

This week your child will bring home a Student Information Verification Form.  The form contains important information such as address, contact phone numbers and email addresses.  We ask all parents to please:

1. Verify that the all information is correct (please correct errors with a pen)
   
2. Sign and date the form and return it to the teacher with your child
   
3. Note that we don’t need the Health Card #, Doctor information and Immunization Information.
   

Please have all forms verified and returned by October 1st, 2021.
